Assessment Specialist

The Assessment Specialist assesses the needs of consumers by rating them in the six core areas of their life by completing the Adult Needs and Strength Assessment (ANSA) / Child and Adolescent Needs and Strength (CANS). The Assessment Specialist completes on-going and as needed client mental health assessments, documenting the services provided, progress or treatment barriers identified on behalf of the client at the clinic, in the client's home or community. The Assessment Specialist functions as a central point of communications and customer service about client progress with other members of the La Familia Del Paso client-centered team.

Rehabilitation Coordinator

The Rehabilitation Coordinator (R.C.) oversees between 5-8 Rehabilitation Specialists. Coordinator empowers Rehabilitation Specialists to provide rehabilitation services to assist assigned individuals in achieving an optimal quality of life including general dependency treatment (COPSD), employment related skills to increase consumers’ independence and ability to maintain community tenure. The R.C. works alongside the Director of Rehabilitation Services to provide training, coaching, and supervision to Rehabilitation Specialists. Team Lead seeks to help specialists achieve all goals set out by the Director.

Medical Assistant

The Medical Assistant (M.A.) assists with the health and well-being of our consumers by providing the best care with excellence for the Mental Health and PCP services that La Familia Del Paso offers. M.A.’s have the important role of making sure consumers get the medications/injection that is usually prescribed by our N.P.’s. M.A.’s are in charge of keeping track of medical records that N.P.’s might need for future follow ups. An M.A. must always be available for our consumers needs and knowing what those needs are what makes us more efficient. 

ICT Rehabilitative Specialist

The Intensive Community Treatment (ICT) program works with individuals to promote optimum mental health and address behavioral health issues of service population. This position works directly with adults to provide individualized treatment in dealing with issues associated with mental and emotional health. The ICT Rehab specialist is responsible for the facilitation of the client’s transition to independence; endorsement and application of community resources and the provision of information and referral services and will provide consumer with evidence- based techniques to improve life/coping skills to aid in acclimating to the community.

Adult Rehabilitation Specialist

The Adult Rehabilitation Specialist is designed to provide rehabilitative services to assist assigned individuals in achieving an optimal quality of life including symptom management, medication education/monitoring, community living skills, chemical dependency treatment, and employment related skills to increase consumer’ independence and ability to maintain community tenure. Provide consumer with evidence- based techniques to improve life/coping skills to aid in acclimating to the community.

Children and Adolescent Rehabilitation Specialist

Children and Adolescent's Rehabilitation Specialist provide rehabilitative services to assist assigned individuals in achieving an optimal quality of life including symptom management, medication education/monitoring, community living skills, chemical dependency treatment, and employment related skills to increase consumer' independence and ability to maintain community tenure (16 years and older). They provide consumers with evidence- based techniques to improve life/coping skills to aid in acclimating to the community. They also provide skills training for parents to assist parent in coping with their child's diagnosis and identified behaviors and help each parent become and advocate for their child. Work with children takes place on weekends and after school. Rehab specialist must be able to accommodate late in day times for appointments (4pm and beyond) based on the child's age and scholastic needs.

Registered Nurse

The Registered Nurse (RN) is responsible for managing the individualized consumer care by promoting and restoring consumer’s health through the nursing process; collaborating with nurse practitioners, and multidisciplinary team members; providing physical and psychological support to consumers.

Enter data from various source documents into an on-line Electronic Health Record (EHR) for storage, processing and data management purposes.

Driver

The Passenger Van Driver provides transportation services to consumers. Drivers will provide transportation to and from La Familia sponsored events, in between different La Familia facilities as needed, and other events as directed. Driver will assist with picking up donations.

Day Program-Qualified Mental Health Professional

Qualified Mental Health Professional will provide short term, intensive treatment to individuals with acute needs that require multidisciplinary treatment in order to stabilize acute psychiatric symptoms or prevent admission to a more restrictive setting. The QMHP will provide a highly structured and safe environment for individuals requiring constant supervision and will ensure an opportunity for frequent interaction between individuals and staff members. QMHP will provide services in a group setting that are goal oriented and focus on reality orientation, symptom reduction, appropriate social behavior, improving peer interaction, improving stress tolerance, and the development of coping skills. The services will assist individuals in achieving an optimal quality of life including symptom management, medication education/monitoring, community living skills to increase consumer’ independence and ability to maintain community tenure as well as provide consumer with evidence-based techniques to improve life/coping skills to aid in acclimating to the community.

Front Desk/Medical Records/Intake Coordinator

La Familia del Paso is looking for a professional Front Desk staff to oversee all receptionist and secretarial duties at several of our front desk locations. incumbent will perform a range of duties including answering phone calls, transferring calls to the appropriate personnel or department, and maintaining the front desk organized and clean. The central goal will be to provide our consumers, their families and all other visitors with outstanding customer service and support. As the ‘face’ of our company, the successful candidate will be presentable and friendly, with outstanding people’s skills. Candidate must have a talent for multi-tasking, with excellent communication and organizational skills.
