Is a Transitional Program for individuals with IDD graduating from High School and transitioning into adulthood.  Many families are limited to resources after high school.  It is our pleasure to have the program as an option for individuals with IDD.   This program provides transportation to and from the facility, provides breakfast and lunch, a full time Licensed Vocational Nurse (LVN) at the location therefore medication monitoring and other services are included.  Members conduct daily activities, arts and crafts, groups, and go on outings occasionally.  Individuals are required to have an IDD diagnosis.  Individuals are required to be Medicaid recipients.  Individuals are not required to complete the Intake Appointment unless the individual is experiencing mental health issues.
